de inquirir se é longe ou perto. O que não atende por estar com visitas, por
ter trabalhado muito e achar-se fatigado, ou por ser alta noite, mau o caminho
ou tempo, ficar longe, ou no morro; o que sobretudo pede um carro a quem não
tem como pagar a receita, ou diz a quem chora à porta que procure outro – esse

The one that doesn't help because he's full of work, or he worked to much and thinks himself tired, because it's late at night, or the weather is not good.
Specially the one who asks something in return from those who cannot pay or deny help to the ones begging for it.
He is not a doctor, he is a Medical businessman, who works to collect the capital and profit from the Graduation costs.
